• You need to fill out the intake form, Covid-19 questionnaire, and liability waiver from the comfort of your own device prior to your appointment. These forms are emailed to you prior to your appointment.
• Please review our Appointment Polices (including Reschedule/Cancellation Policy) .
• Please give us a call if you have any questions about your upcoming appointment.
